CrowdHealth Membership
When you enroll for a CrowdHealth membership through our Site, we ask for your personal information, including your first and last name, email address, phone number, date of birth, zip code, employment status, sex, height, weight, tobacco usage, your perceived health status, current health coverage status, whether you have any preexisting medical conditions, and whether you are pregnant. If you enroll members of your family, we will ask you for the above information for each family member you enroll.
Communications
If you contact us directly, we may receive personal information about you. For example, when you contact us for more information about CrowdHealth or our services, we will receive your first and last name, email address, the contents of your message, and any additional information you choose to provide. If you sign up for our newsletter, we will ask you for your first and last name and email address.
Careers
If you apply for a job with us through a third-party platform (such as Indeed),we will collect the personal information you make available to us through thatthird-party platform.
Making a Payment
When you make a payment through our Site, your payment card information or bank account information, billing information, and any other financial information necessary to complete your payment (“Payment Information”) is processed by our third-party payment partners and we do not collect, store, or process your Payment Information. When you use the Site, we may infer your general location information (for example, your IP address may indicate your general geographic region).
